To recover a lost password…
- Open your Patient Rewards Hub app
- Click on the “Forgot Username or Password?” link at the bottom of the login screen.
- Type your 16-digit username or patient email address into the field provided
- Click “Send Email”
- Once you receive the email, follow the instructions it contains to reset your password.
What if there is no email associated with your account?
If you have not connected a patient email with your Patient Rewards Hub account, please call 1-800-560-1469 ext. 3 for password reset assistance.
